Claim Form (Step 1 of 4)

In order for you to qualify to receive a payment related to Stokes v. NovelPay, LLC, as described in the Notice of this Settlement (the “Class Notice”), you must file a Claim Form.

Your claim will be considered only if you comply with all of the following conditions:

  1. You must be a person to whom a Class Notice was addressed, and you must be listed as a Settlement Class Member on the Stokes v. NovelPay, LLC Class List. If you have a question about whether you are listed as a Settlement Class Member, please contact the Settlement Administrator at (833) 319-2034.
  2. You must complete this Claim Form.
  3. By submitting the Claim Form, you are certifying that you are a member of the Class in the Stokes v. NovelPay, LLC case.
  4. If you do not complete and electronically submit or mail the Claim Form by August 8, 2025, you cannot receive any payment from the Settlement. So that you will have a record of the date of your mailing of the Claim Form and its receipt by the Settlement Administrator, you are advised to use (but are not required to use) either the Settlement Website or U.S. Mail by Certified Mail, Return Receipt Requested.

Your Claim Number and PIN Number must be entered exactly as they appear on your Mailed or Email Notice.

If you have not received a Mailed or Email Notice with a pre-assigned Claim Number but believe that you are a Settlement Class Member of this Lawsuit, please contact us.
